How to order

  1. Complete sections 1 and 2 of the ResponseDX® requisition with physician/patient information.
  2. Attach copies of the pathology report and patient insurance information (or complete sections 3 and 4).
  3. Check appropriate panel or individual genes in section 5.
  4. Sign physician’s certification.
  5. Fax the completed form, pathology report and insurance information to Response Genetics, Inc. at (323) 224-3096.

Response Genetics will request the patient specimen from pathology, analyze the specimen and return a report to the physician within 5–7 days of receipt of the sample.
